Abstract
Introduction Changes in psoriatic arthritis (PsA) disease activity, particularly flares, are unpredictable and can impact patients’ quality of life. Digital biomarkers derived from data collected via smart devices provide an opportunity for unobtrusive continuous monitoring of symptoms reflecting disease activity. Besides, our understanding of the mechanisms and triggers behind flares is limited. The primary objectives of the Psoriatic Arthritis Digital Phenotyping and Inflammation Drivers (PDPID) study are: (1) to develop digital biomarker capable of detecting PsA flare using data collected from patient’s smartphone and smartwatch, and (2) to develop machine learning models for PsA flare prediction using clinical, biological, environmental and digital data.
Methods and analysis The PDPID study is a 12-month multi-centre prospective cohort study conducted across four countries. Study visits are scheduled at baseline (T0), and 3, 6, 9 and 12 months follow-up visits, with additional patient-initiated visits due to flare. At inclusion, patients have a study app installed on their smartphone and receive a smartwatch. Flare information is collected using physician- and patient-reported flare questionnaires administered at each visit, and via the flare button in the app that patients can activate. The study collects clinical data (physical measurements, questionnaires), biological markers (salivary DNA, gut microbiome, hair cortisol, C-reactive protein) and environmental exposure data (weather and air pollution). The app includes in-app questionnaires, active photo and video tests and passively captures digital data from the smartphone and smartwatch sensors.
Ethics and dissemination Ethical approval has been granted from each of the participating countries (Erasmus MC, the Netherlands: MEC-2023-0470; HRA and Health and Care Research Wales, UK: 332916 NOVA Medical School|Faculdade de Ciências Médicas (NMS|FCM) Ethical Committee (CEFCM), Portugal (124/2023/CEFCM); MREC Hipokrateion Hospital Thessaloniki, Greece: 5549/31.01.24). Findings of this study will be disseminated through reports to the funding body, the project website, newsletters, social media, national and international conferences and symposiums, and scientific publications.

20,634 sand flies were collected, including 13,676 females (66.3%), of which 4635 samples representing 5050 females were analyzed. Fourteen sand fly species were identified. Larroussius species dominated, with Ph. syriacus prevalent in Aderet (27.5%), while Ph. galilaeus (54.1%) and Ph. tobbi (24.9%) predominated in Kahal. Three samples were positive for L. infantum DNA: two individual unfed female Ph. syriacus from Aderet, and one pooled sample from Kahal. These findings represent the first species-resolved detection of L. infantum DNA in Ph. syriacus in Israel and worldwide. Blood-meal analysis identified vertebrate hosts in 74.2% of engorged females, with strong dominance of cattle (84.8%), followed by canids (5.8%) and other mammals. Phlebotomus syriacus were found to feed on canids (16.7%), supporting its potential epidemiological relevance.

In today's world, the number of global nuclear weapons has exceeded the amount needed to destroy the earth, but it is very difficult to reduce and eliminate them. After the Russia-Ukraine war, Europe and the United States are worried about Russia's nuclear weapons. The power of the Zaporizhzhia nuclear power plant is ten times that of the Chernobyl's. However, it has been repeatedly attacked. In case of a full-scale nuclear explosion, the disaster coverage area will be about 512 million square kilometers, which will exceed the total area of the earth. Human cells around the world will be harmed. The two atomic bombings in Hiroshima and Nagasaki in 1945 should have paralyzed at least 20% of Japan and made it uninhabitable for humans for at least 100 years, but the opposite is true. Just based on this scientific logic, it is sufficient to prove that the US military and Japan cooperated to successfully carry out a nuclear psychological war at that time. The United States has spent $7.2 trillion on nuclear weapons. Japan did not point out the wrong investment by the United States and has long enjoyed the dividend of being a so-called atomic bomb victim. If the U.S. had entrusted Buffett's team with investing only $100 billion in securities related to the S&P 500 in 1965, investment income would have risen to $2472.627 trillion by 2018, and there would have been no debt crisis. Moreover, the United States would at any time have had enough funds to begin building 100,000 kilometers of high-speed rail around the United States and high-speed rail reaching the Bering Strait. In order to escape nuclear destruction and promote normal economic development, the public should be aware of the above-mentioned atomic bomb psychological war.
