Maqolada bolalarda tish kariyesining rivojlanishiga ta’sir etuvchi biologik, xulq-atvor, ovqatlanish va ijtimoiy-iqtisodiy omillar hamda zamonaviy profilaktika usullari dalillarga asoslangan stomatologiya nuqtayi nazaridan tahlil qilindi. Tadqiqot birlamchi klinik tajriba emas, balki Jahon sog‘liqni saqlash tashkiloti ma’lumotlari, 2023–2026-yillarda chop etilgan sistematik sharhlar, meta-tahlillar va O‘zbekiston mualliflarining bolalar stomatologiyasi bo‘yicha ilmiy ishlariga asoslangan ikkilamchi dalillar sintezidir. WHO ma’lumotlariga ko‘ra, davolanmagan sut tishlari kariyesi dunyo bo‘yicha 514 million bolaga ta’sir qiladi. 2024-yilgi oziqlanish omillari meta-tahlilida noto‘g‘ri oziqlantirish amaliyoti erta bolalik kariyesi (ECC) bilan OR 3,64, yuqori shakar iste’moli OR 3,24 va meva-sabzavotning kam iste’moli OR 2,71 darajasida bog‘langan. Tungi emizish bo‘yicha boshqa 2024-yilgi meta-tahlilda ECC xavfi RR 2,35 bo‘lgan. Profilaktikada ftorli tish pastasi, professional ftorli lak, fissura germetiklari, silver diamine fluoride (SDF), ota-onalarni o‘qitish va riskga asoslangan recall tizimi asosiy yo‘nalishlar sifatida ajratildi. Ftorli lakning klassik pooled prevented fraction ko‘rsatkichi doimiy tish sirtlarida 46% va sut tishlarida 33% bo‘lgan; 2025-yilgi zamonaviy review esa 14 RCT va 13 mingdan ortiq bolada uning xavfsiz va samarali ekanini tasdiqlaydi. Fissura germetiklari doimiy molyarlarda 36 oygacha kariyes xavfini kamaytirgan, SDF bo‘yicha 2024-yilgi sistematik sharhlar esa kariyesni to‘xtatishda izchil foyda ko‘rsatgan. Natijalar bolalar kariyesi profilaktikasi bitta muolaja emas, balki shakar ekspozitsiyasini kamaytirish, ftor, germetik, SDF va ota-ona ishtirokini birlashtiruvchi ko‘p bosqichli model bo‘lishi kerakligini ko‘rsatadi.

Chronic material pressure can constrain not only the objective opportunity structure available to individuals but also their capacity to generate, compare, elaborate, and revise alternative courses of action. Existing approaches often risk conflating objective opportunity, perceived opportunity, behavioral execution, and the cognitive capacity to represent alternatives. This paper develops a dynamic state-space model that separates these dimensions and specifies how they may evolve over time.   The model is organized around two analytically distinct state variables: objectively audited opportunity structure, O_objective, and Strategic Imagination Capacity (SIC). SIC is defined as the capacity to generate, elaborate, compare, and revise representations of future alternative courses of action under active constraint. Perceived opportunity, rehearsal, recovery/cognitive availability, and observable execution are treated as distinct measurement or process variables rather than as substitutes for either state variable.   The original sequential three-stage conception is reformulated as a dynamic state space. Stage 1 represents a narrow but real opportunity margin with preserved SIC; Stage 2 represents partial erosion of comparison capacity; Stage 3A represents substantial SIC erosion while objective opportunity may remain open; and Stage 3B represents objective opportunity closure with comparatively preserved SIC. A joint closure configuration, J, represents simultaneous low objective opportunity and low SIC without constituting a separate stage.   The paper specifies hypotheses concerning closure exposure, rehearsal, recovery, individual sensitivity, objective/perceived opportunity dissociation, behavioral equifinality, recovery trajectories, and temporal persistence. A longitudinal, multi-source observational design is proposed with four principal measurement waves over 18 months and higher-frequency measurement of closure exposure and process variables. A Bayesian hierarchical dynamic model is specified together with simulation-based power analysis, kernel comparison, measurement validation, and pre-specified decision rules. As of the present version, the simulation stage of the protocol has been executed in full — five simulation experiments covering identifiability, parameter recovery, recovery boundaries, statistical power, and state-classification validity.   The model does not claim that chronic material pressure inevitably produces strategic closure, nor does the design establish causal effects, population prevalence, or persistence beyond the observation window. Its primary contribution is a testable structural separation between objective opportunity, represented opportunity, strategic capacity, process exposure, recovery conditions, and observable execution.   Manuscript status: Conceptual architecture, hypotheses, operationalization, study design, analysis plan, decision rules, discussion, limitations, and conclusion are closed. Simulation stage executed (v0.2.0-simulation, 2026-09-26) — 5 experiments completed, ~180 automated tests passing, documentation cross-verified against CSV artifacts. Cardiovascular autonomic dysfunction is one of the most significant non-motor manifestations of Parkinson's disease (PD). Orthostatic hypotension (OH) holds a particular place in this context; it can be either symptomatic or asymptomatic and is associated with falls, impaired daily living activities, and reduced quality of life. The aim of the study was to assess the prevalence of orthostatic hypotension, the characteristics of its clinical manifestations, and its relationship with the duration and severity of Parkinson's disease. A single-center observational study was conducted involving 120 patients with a clinical diagnosis of Parkinson's disease. Patients with OH exhibited a more advanced mean disease stage, longer disease duration, and more pronounced motor and non-motor impairments. Orthostatic hypotension is a common non-motor manifestation of PD and can be observed even at relatively early stages of the disease. As PD progresses, the prevalence of OH and the severity of the orthostatic drop in blood pressure increase. Given the potential for an asymptomatic course, measuring blood pressure in both the supine and upright positions should be included in the routine neurological assessment of patients with PD.
