Современная терапевтическая концепция предусматривает мультимодальный подход с индивидуальным сочетанием образовательных мероприятий, сохранения физической активности, медикаментозной терапии, реабилитации и, при наличии показаний, интервенционных и хирургических методов. При этом эффективность отдельных фармакологических групп остаётся неоднозначной, а наиболее современные сетевые метаанализы характеризуют качество доказательств по большинству нехирургических вмешательств как низкое или очень низкое. Целью исследования является провести клинико-статистическую оценку эффективности оптимизированной мультимодальной терапии пациентов с острой пояснично-крестцовой радикулярной болью и определить факторы, ассоциированные с уменьшением болевого синдрома и функциональных ограничений. В исследование включены 120 пациентов с острой пояснично-крестцовой радикулярной болью продолжительностью до 12 недель. Клинически значимое уменьшение боли ≥50% отмечено у 70,0% пациентов основной группы и у 48,3% пациентов группы сравнения (p=0,017). Между снижением интенсивности боли и улучшением функционального состояния выявлена выраженная положительная корреляционная связь (r=0,61; p<0,001). Современные исследования подчёркивают необходимость выделения нейрогенной ортостатической гипотензии (нОГ), обусловленной нарушением автономной регуляции сердечно-сосудистой системы, поскольку её клиническое значение отличается от вторичной ортостатической гипотензии. Целью исследования является оценить частоту ортостатической гипотензии, особенности её клинических проявлений и характер связи с длительностью и тяжестью болезни Паркинсона. Во время одноцентрового наблюдательного исследования рассмотрены 120 пациентов с клинически диагностированной болезнью Паркинсона. Проводилась оценка двигательных и немоторных проявлений заболевания, стадии по шкале Hoehn–Yahr, показателей артериального давления и частоты сердечных сокращений в положении лёжа и после вертикализации. В выборке ОГ выявлена у 41 (34,2%) пациента, из них у 30 (25,0% общей выборки) предполагался нейрогенный характер. Частота ОГ возрастала по мере увеличения стадии заболевания: с 16,0% на I–II стадии до 64,0% на IV стадии Hoehn–Yahr. Между стадией заболевания и выраженностью ортостатического падения систолического АД выявлена умеренная положительная корреляция (r=0,46; p<0,001). Средняя стадия Hoehn–Yahr у пациентов с ОГ составила 2,9±0,1, без ОГ — 2,1±0,1; различия были статистически значимыми (t=5,3; p<0,001).

Основное внимание уделяется профессионально-коммуникативному контексту как важному условию функционального усвоения медицинской терминологии. Анализируются положения коммуникативного подхода, English for Specific Purposes (ESP), а также результаты исследований в области профессиональной и клинической коммуникации.На основе анализа научно-методической литературы уточняется содержание терминологической компетенции применительно к обучению медицинскому английскому языку. Владение медицинской терминологией рассматривается не только как знание специальных терминов, но и как способность осознанно отбирать и использовать их с учетом профессиональной задачи, клинического контекста, цели общения и участников медицинского дискурса.Отдельное внимание уделяется различиям между профессиональным общением медицинских специалистов и взаимодействием «врач–пациент». Обосновывается необходимость рассматривать медицинский термин как функциональный компонент профессиональной коммуникации и соотносить его усвоение с типичными ситуациями медицинского общения.
