EARLY DIAGNOSIS AND IMPROVEMENT OF TREATMENT EFFECTIVENESS OF COGNITIVE IMPAIRMENT IN CHRONIC CEREBRAL ISCHEMIA

Background. Chronic cerebral ischemia (CCI) is a persistent reduction of cerebral blood flow caused by atherosclerotic large-artery disease and cerebral small vessel disease. It is a frequent substrate of vascular cognitive impairment, yet its cognitive consequences are usually recognised only once they have become disabling.
Objective. To summarise evidence on the early diagnosis of cognitive impairment in CCI and on the determinants of treatment effectiveness, and to convert that evidence into a pathway applicable at the bedside.
Methods. Narrative review of guidelines, systematic reviews, imaging studies and randomised controlled trials retrieved from PubMed and Europe PMC, with every quantitative statement referred to its primary source.
Results. Cognitive impairment in CCI develops through hypoperfusion-driven neurovascular unit dysfunction, myelin injury in white matter that still appears normal on conventional imaging, and disconnection of subcortical–cortical circuits. A lower cerebral blood flow predicts later decline independently of lesion burden and atrophy. Early detection therefore rests on risk-based screening with the Montreal Cognitive Assessment and confirmatory magnetic resonance imaging, supported where available by perfusion imaging, advanced diffusion metrics, electrophysiology and blood markers. Treatment evidence is strongest for vascular risk-factor control: blood-pressure lowering reduced cognitive decline by 19% in PROGRESS and incident mild cognitive impairment by 19% in SPRINT MIND. Symptomatic drugs give small cognitive gains, whereas a multidomain programme improved cognition in FINGER and a 14-week exercise programme neither raised perfusion nor improved cognition.
Conclusion. Effectiveness is increased chiefly by shortening the interval between vascular risk exposure and diagnosis, treating the modifiable vascular substrate before injury becomes irreversible, and combining pharmacological with lifestyle intervention under objective monitoring.

This is the broad reading guide and preservation collection for modern LaTeX transcriptions and translations of historical mathematics. It brings together European, Chinese, Indian and Islamic mathematical texts, original-language editions, translations, editable sources, source witnesses, editorial apparatus and checking records. Historical authors remain the authors of the mathematics; modern editorial and model credits belong to the individual editions.
The collection has mixed quality and completeness. Some bounded works have completed documented source checks; large working readers, partial checkpoints and known defective drafts are also preserved. Start with the PDF reading guide in the preview. It identifies the stronger current reading paths, explains their actual coverage and review limits, and links the exact current project editions. A successful build, an attractive PDF or a filename containing Complete is not an accuracy certificate.
Useful starting points: Gordan's Thetafunctionen and De linea geodetica, Dirichlet Band I Paper IV, Sylvester Volume I, the source-available Steinitz corpus, Kneser's Variationsrechnung, Bianchi's Differential Geometry I, and the FAC and GAGA editions have documented bounded scopes and checking evidence. The guide gives the qualifications for each. Current Noether, EGA, SGA, Deligne and Weber readers remain available with their individual correction and review histories.
Gauss is ongoing: the separate Gauss record now includes the dated source, literal English and editorial collections. Band I's latest local review reaches physical page 327 but the assembled reader retains a gap at pages 80-231. Band II's principal reviewed reader reaches physical page 453; pages 454-457 remain a draft. Neither volume is complete. The current production priority is to complete I and II, including missing coverage, ordered integration and final reviews, and then continue to later Gauss volumes.
Known limitations remain visible: Cayley files with substantial symbol and text mismatches remain repair/provenance material, not recommended source-faithful editions. Maxwell, Gibbs, Dedekind and Poincare retain explicit partial coverage or gaps. The al-Battani guide points to the direct corrected notes and records the older package mismatch. Broad multilingual coverage is not a claim of uniform source checking or native-human review. Source statements, translations and substantive editorial corrections must remain identifiable.
Edition update, 3 October 2026: the new 13-page guide reconciles 33 public project records, including newer dedicated EGA and SGA language editions and the new Gauss supplement. Its companion ZIP contains editable prose, a reproducible PDF recipe, status data, observed public metadata and file inventories. The 98 files of the preceding landing edition are retained byte-for-byte. Older July SGA 2 checkpoints and bulk preservation archives remain historical; current satellite readers are linked in place rather than duplicated into this landing record.
The guide reports published evidence and disclosed limitations; it is not a new independent review of every source page, translation or theorem. Original research, Lean formalization and workflow/methodology records have separate entries and separate evidentiary scope. The canonical Split-Zero research family remains 10.5281/zenodo.22678085. Artifact-specific rights and human-source attribution remain in force; this landing edition grants no new rights over underlying texts.

The canonical current package is Time_Crystals_Review_Data_v1_1.zip, version 1.1.0, created by Habib Hamam. It supersedes the analytical data and creator attribution in Zenodo_Time_Crystals_Data_v1.zip. The older files are retained for provenance. Search cutoff: 30 September 2026.
The current package contains the 120-report evidence map (35 experimental, 85 analytical/numerical), screening ledgers, search documentation, correction and bibliography audits, reproducible counts and figure inputs. Positive R/P/A diagnostic counts are 8/17/12. Source-linked extraction and experimental-evidence qualifications accompany the corrected coding.
Original PaperGuide exports comprise 508 references, 508 title/abstract decisions, 118 full-text decisions and 49 extraction records, plus saved protocol definitions and a DOI crosswalk. Forty-eight original extraction records match included reports. These exports precede subsequent eligibility corrections and supplementary searches.
Coverage is incomplete for the remaining 72 reports. Software-generated evidence passages and confidence scores are not independently recertified quotations or certainty grades. False diagnostic flags mean evidence was not identified in the coded source, not that a test was absent. Finite observation windows are distinguished from fitted coherence times and numerical lifetimes. These are review data, not raw experimental measurements; article PDFs are not redistributed.
