The environmental crisis in the Aral Sea region has created a uniquely hazardous habitat that profoundly affects pediatric health. This study aims to evaluate the clinical and trophological status alongside the metabolic profile of children suffering from recurrent respiratory diseases who reside in the epicenter of the Aral Sea ecological disaster. The research involved a comprehensive clinical, anthropometric, and biochemical examination of one hundred and twenty children with recurrent respiratory tract infections, compared against a control group of fifty healthy children from the same region. The methodology strictly adhered to the standardized criteria for assessing physical development and metabolic markers. The results demonstrated a significant lag in the physical development of the main group, characterized by a high prevalence of stunting and reduced body mass index. Biochemical analysis revealed pronounced metabolic disturbances, including hypoproteinemia, dyslipidemia with elevated atherogenic fractions, and severe deficiencies in essential trace elements such as zinc and iron. These metabolic shifts indicate a state of chronic cellular hypoxia and oxidative stress induced by continuous exposure to environmental toxicants. The study concludes that recurrent respiratory diseases in this specific ecological zone are closely linked to profound trophological and metabolic disorders, necessitating the development of targeted, region-specific rehabilitation and nutritional intervention programs.

This article offers a rigorous art-historical investigation into the evolutionary dynamics, geometric typologies, and structural composition of ornamental decorations within traditional Uzbek copper engraving during the 19th and 20th centuries. Following the historical synthesis of Islamic iconographic principles and preexisting Central Asian visual practices, the study traces how early narrative-figurative depictions underwent a conceptual shift toward highly stylized, abstract configurations. Utilizing the foundational classification methodologies of modern scholarship, the research categorizes these decorations into four dominant typologies: geometric (girih), vegetative-floral (islimi), zoomorphic, and calligraphic-epigraphic systems. Furthermore, it deconstructs six core structural layouts utilized on flat surfaces: gardish, davra, turunj, mehrob, sitora, and ishkalak. The findings demonstrate that traditional Uzbek metal decoration operates on a strict law of bounded spatial borders and modular balance, functioning as an essential indicator of regional identity, socio-economic standing, and cultural continuity.

Congenital titinopathies, arising from biallelic truncating, splice-altering, or metatranscript-only variants in the TTN gene, typically present with hypotonia, arthrogryposis, feeding difficulty, and respiratory insufficiency. Cardiac risk in this group is variant-position-dependent and age-penetrant rather than uniform, and presentations dominated by bulbar dysfunction without cardiac involvement remain sparsely documented. We describe a 12-year-old girl of Irish Traveller descent with a clinical diagnosis of congenital titinopathy and arthrogryposis multiplex congenita (AMC); biallelic TTN truncating variants were reported in early childhood, but the primary molecular report was not retained and could not be recovered, leaving the variant position and its predicted effect on cardiac isoforms unknown. Her phenotype is marked by severe bulbar impairment manifesting as persistent oropharyngeal dysphagia, recurrent aspiration, and gastrointestinal dysmotility, in the absence of cardiac or pharyngeal anatomical abnormalities. She is BiPAP-dependent and PEG-fed, with only intermittent oral intake due to aspiration risk, and has undergone multiple hospitalisations for aspiration pneumonia, faecal loading, and suspected sepsis. Despite repeated episodes of respiratory decompensation, cardiac structure and function have remained normal on serial echocardiography and ECG across more than a decade, and surveillance is continuing. A sibling reported to share the same TTN genotype demonstrates only distal weakness, underscoring notable intrafamilial phenotypic variability. This case documents a bulbar-dominant congenital titinopathy without cardiomyopathy to date, while illustrating that the loss of molecular records precludes genotype-based cardiac risk stratification and therefore does not permit surveillance to be relaxed. The patient's sociomedical context, including caregiver burden and structural barriers to care, further emphasises the need for culturally contextualised genomic services and longitudinal multidisciplinary support in underserved populations.

Abstract. We analysed 366 neonatal deaths and 300 in-hospital infant deaths recorded in Fergana region in January–June 2025 and 2026, together with 385,030 paediatric hospital treatments in 2024–2025. Neonatal mortality rose from 4.0 to 4.4 per 1,000 live births (RR 1.11; 95% CI 0.90–1.36; p=0.35). Late neonatal deaths increased from 57 to 70. Deaths within 24 hours of admission rose from 4.1% to 7.2% (p=0.32). Neither change was statistically significant, but both should be reviewed routinely.

The study examines how public servants' implicit citizenship theories (ICTs) shape their willingness to collaborate with citizens, using a scenario experiment combined with a single-profile conjoint design fielded among public servants in Germany (n = 379) and Taiwan (n = 365). The dataset comprises 2,976 candidate-profile evaluations.
The record includes the de-identified survey and experimental data (one row per evaluated profile) and the Stata do-files used for data preparation and analysis, which reproduce all tables and figures reported in the article and its online appendices. A codebook describing all variables is included.

This preprint develops a task-relative framework for determining which distinctions in a finite stochastic state must be retained to preserve specified decisions through time. Building from instantaneous task equivalence, it examines when a task-relevant quotient remains closed under stochastic evolution and introduces dynamic task closure by pulling future task directions backward through the dynamics. The construction extends to changing tasks, finite horizons, and uncertain future dynamics. A three-state example illustrates how a presently task-invisible state direction can become dynamically visible before a decision-boundary crossing occurs. The manuscript further distinguishes natural decision evolution from compression-induced distortion and develops margin-based certificates for decision preservation, including temporally and directionally aligned refinements. Randomized stress tests are used as implementation checks and to examine certificate conservatism. The proposed contribution is a decision-relative synthesis connecting dynamic task relevance, future task closure, compression, and decision preservation while remaining explicitly grounded in established work on observability, Markov-chain lumpability, invariant subspaces, and model reduction.
